Schedule an Appointment
Please call or visit the dental clinic to schedule an appointment. Appointments made over the phone require that the soldier’s dental record be available at the dental clinic at the time of phone call. Appointments are generally scheduled no more than 3 weeks ahead of time. Appointments may be canceled or rescheduled via phone or in person; however, patients with Dental Readiness Classification 3 (appearing “amber” in Medpros for Dental Readiness) require a phone call or visit from their 1SG or Commander in order to cancel or reschedule a dental appointment.
Failed Appointment Policy
All Soldiers are responsible for being on time for all scheduled appointments. If it is necessary to reschedule or cancel an appointment, the Soldier is responsible for contacting the Dental Treatment Facility (DTF) as soon as possible but no later than 24 hours prior to the scheduled appointment time. Please note that Soldiers may NOT cancel their own Class 3 Dental appointments; ONLY the First Sergeant or Commander can cancel a Class 3 Dental appointment.
A Soldier who does not attend an appointment without giving 24 hour notification to the DTF will be notified via email along with their First Sergeant and Commander. Soldiers who fail a second appointment must be accompanied by their first line supervisor to the DTF to be counseled by the Clinic NCOIC or OIC prior to scheduling an appointment. Soldiers who fail a third appointment will not receive non–emergency dental care from the DTF without counseling from their Commander or First Sergeant, and must be escorted by their First Sergeant to receive a dental appointment.
All Soldiers have been given a copy of the DENTAC Failed Appointment Policy Letter #16 to review and sign; this letter explains the Failed Appointment Policy in full and is retained the Soldier’s dental record for reference.
Directions to the Clinic
Approaching Fort Hood from Hwy 190 via TJ Mills Main Gate Exit, enter through Main Gate and proceed straight on TJ Mills Blvd. Make a left at 4th traffic light onto Battalion Ave. Perkins Dental Clinic will be on the left, past 72nd Street, adjacent to Monroe Troop Medical Clinic. If entering Ft. Hood via Clear Creek Gate, make a right onto Battalion Avenue, Perkins Dental Clinic will be on the right.
