Our goal is to assist patients through all phases of illness and recovery, assuring a smooth transition in returning to independence and managing their own health care.

Case Management is a process that involves a team of health care professionals who help patients and families gain control over their illness, injury or situation. It is patient-centered rather than disease specific and is coordinated by a case manager assigned to a particular patient. The medical case management program consists of registered nurses and an administrative assistant. The patient’s primary doctor, nurse practitioner or physician assistant are also part of the team that helps make plans and decisions about the patient’s health goals.

Many patients with very complex medical conditions or situations require case management. Examples of patients needing case management are those with multiple specialty providers or multiple disease processes. Patients who feel they need assistance concerning their medical care may benefit from case management services.

Providers can submit a referral via the patient’s electronic medical record or a patient can self-refer. The request will be reviewed by the case manager and considered for acceptance.

Population Health case managers are available when a patient’s clinic does not a have a designated case manager. We are located in Population Health on the first floor of CRDAMC. Our contact number is (254) 286-7467.
