Getting your Physical status into your personnel record:

Physical exams can be updated by the Soldier Medical Readiness Center through the MEDPRO system.
A small portion of the Part II Physical Exam includes a section for the soldiers physical profile (PUHLES). For those soldiers undergoing a Periodic physical, an additional memo is generated at the time of the exam. This memo is completed by the medical officer reviewing the exam and reflects the PUHLES status of the soldier.
The SMRC enters the data directly into the MEDPRO system and it usually takes approximately 48-72 hours once the physical is approved.
The SMRC maintains a copy of the physical exam for one year.
