The Thomas Moore Health Clinic is a Soldier Centered Medical Home servicing only Active Duty Soldiers.
The Soldier Centered Medical Home model is both revolutionary and old-fashioned. Each Soldier partners with a team of healthcare providers physicians, nurses, behavioral health professionals, pharmacists, and others to develop a comprehensive, personal healthcare plan. The healthcare team works with each patient over time to take care of health issues as they arise, ensure delivery of prevention screening and services, manage chronic conditions, and promote a spirit of health, wellness and trust.
At its heart, the Soldier Centered Medical Home of TMHC is healthcare the way it should be easy to access, patient-centered, team based, and quality focused.
Thomas Moore Health Clinic is a full service facility, including pharmacy, radiology and lab. It also houses the Allergy Clinic, Department of Optometry, and the Soldier Medical Readiness Center.
Active Duty Sick Call:
Monday – Friday, Sign In 6:30 a.m. – 7:30 a.m.
- Must have DA Form 689 signed by Unit.
- Active Duty Sick Call is for acute or urgent illnesses.
- For urgent issues after Sick Call, please walk into the clinic for triage evaluation.
- Uniform for Sick Call or Active Duty appointments is BDUs/ACUs.
- No APFT Uniform unless you are here for a physical.
- Active-duty pregnancy sick call is at the Women's Health Center, Carl R. Darnall Army Medical Center. OB sick call sign–in is 7:30 a.m. – 8 a.m., Monday – Friday.
- Please get a Sick Slip (DD Form 689) from your unit.
- For care after duty hours/weekends go to CRDAMC Emergency Department.
